29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: (a) On or about 02/15/2017 - 499 Boeing Boulevard, Huntsville, AL, the south wall collapsed on an employee hand digging for fiber optic cable in a trench excavation measuring approximately 27 feet long x 15 feet, 3 inches wide, and 4 feet, 6 inches in depth.
