29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. On or about March 1, 2017 at the facility located at 900 Grossman Dr. Schofield, WI employees were exposed to the point of operation created between the dies of an unguarded Amada HDS1303 NT hydraulic press brake located in the production department. b. On or about March 1, 2017 at the facility located at 900 Grossman Dr. Schofield, WI employees were exposed to the point of operation created between the dies of an unguarded Amada HDS2204 NT hydraulic press brake located in the production department. c. On or about March 1, 2017 at the facility located at 900 Grossman Dr. Schofield, WI employees were exposed to the point of operation created between the dies of an unguarded Amada HDS1303 NT hydraulic press brake located in the production department.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. a. On or about March 1, 2017, the employer located at 900 Grossman Drive Schofield, WI 54476, did not verify that the required workplace hazard assessment had been performed for employees exposed to fumes while welding galvanized steel.
