29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:    a) Facility, where electrical equipment not used as listed or labeled included:   1. Meat Department Change and Supply Room, where a relocatable power tap was used to supply power to appliances including, the Mr. Coffee drip coffee pot. (Item Corrected)   2. Carry Out Personnel Break Room, where relocatable power taps were used to supply power to appliances including, the GE microwave oven, the Hamilton Beach microwave oven, and the Oster Toaster oven. (Item Corrected)   3. Administrative Cubicle, where the Eaton uninterrupted power source (UPS) relocatable power tap, the Leviton relocatable power tap and the Ace relocatable power tap were daisy chained and used to supply power to computer equipment.  The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 1169973, citation number 1, item number 1 and was affirmed as a final order on September 28, 2016 with respect to a workplace located at Marysville, WA.  The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 1122685, citation number 1, item number 1 and was affirmed as a final order on April 8, 2016 with respect to a workplace located at Joint Base Lewis Mc Chord, WA.

29 CFR 1910.303(g)(1)(ii): Working space for equipment likely to require examination, adjustment, servicing, or maintenance while energized was used for storage:   a) Facility, where space located in front of electrical panels used for storage included:  1. Cashier Office, where items including two filing cabinets, books, and a telephone were stored in front of circuit panel CR.  2. Server Room, where items including boxes containing toner cartridges, empty cardboard boxes, and mailing packages were stored in front of circuit panels GG Left, GG Right, and CRI.  3. Battery Charging Area, where the Encore Floor Scrubber was stored in front of circuit panel FF.   The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(g)(1)(ii), which was contained in OSHA inspection number 1122685, citation number 2, item number 1 and was affirmed as a final order on April 8, 2016 with respect to a workplace located at Joint Base Lewis Mc Chord, WA.

Yale Forklifts, where the Ansul Sentry fire extinguishers were equipped with tags dated 9/2.  4. Loading dock Break Room, where the Amerex Fire extinguisher was equipped with a tag dated 9/2.   5.
