29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors from sources such as open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, mechanical sparks and spontaneous ignition:    (a)  Gasoline Transfer at 242 South Hull Street, Sinking Springs, Pennsylvania and Lot including Garage - The employer reclaimed gasoline (a Category 2 flammable liquid) from the fuel tanks of salvage vehicles.  Employees use a ferrous metal pick axe to puncture a hole in each metal fuel tank, which could generate sparks, exposing employees to the hazards of fire and explosion, on or about February 22, 2017.    (b)  Gasoline Transfer at 242 South Hull Street, Sinking Springs, Pennsylvania and Lot including Garage - The employer transferred reclaimed gasoline (a Category 2 flammable liquid) from the salvage vehicle's fuel tanks, into a tank on the ground.  The employer did not ensure that the car's fuel tank was bonded to the capture tank and did not ensure that the capture tank was grounded to dissipate static charges generated by the flow of the flammable liquid, exposing employees to the hazards of fire and explosion, on or about February 22, 2017.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.157(c)(4): The employer did not assure that portable fire extinguishers were maintained in a fully charged and operable condition and kept in their designated places at all times except during use:    (a)  242 South Hull Street, Sinking Springs, Pennsylvania and Lot including Garage - Portable fire extinguishers were not maintained near the gasoline draining process, and were not fully charged.  They were not located for easy access, thereby exposing employees to injuries on or about February 22, 2017.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.178(m)(2): Employee(s) stood under or passed under the elevated portion of a powered industrial truck:    (a)  Gasoline Transfer at 242 South Hull Street, Sinking Springs, Pennsylvania and Lot including Garage - The Employees lifted scrap vehicles with a Hyster forklift and stand under the forks while suspending the vehicle to puncture a hole or drill a hole into each fuel tank , exposing employees to the hazards of crushing, on or about February 22, 2017.      Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Examinations were not made at least daily:  (a)  Gasoline Transfer at 242 South Hull Street, Sinking Springs, Pennsylvania and Lot including Garage - The employer uses a Hyster forklift to lift cars to reclaimed gasoline from the fuel tanks and parts from salvage vehicles and din not inspect the forklift daily before use, thereby exposing employees to injuries, on or about February 22, 2017.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
