29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On or about December 17, 2016, the employer did not ensure employees utilized the procedures for the safe application, usage, and removal of potentially hazardous energy controls while splicing bail wires in the chamber of the Machinex Baler. Energy sources include hydraulic and electric power.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):(a). On or about February 22, 2017, a baler machine operator was exposed to machine hazards associated with the unexpected energization of moving parts while servicing the bailing wire inside the bale chamber of a Machinex baler machine. The employer did not ensure the baler machine operator implemented hazardous energy control procedures by turning off the main disconnect switch on the machine control panel [per the 1910.147(d)(2) requirements], isolating the energy sources by operating the master control relay (A key) [(d)(3)], affixing a lockout device (lock) on the main control disconnect switch [(d)(4)(i)], and verifying the baler was de-energized [(d)(6)].
