On 2/22/2017 employees from Angel Martinez-Cabrian were replacing roofing materials on the roof of a residence located at 1349 Cave Springs Estates Drive, St. Peters, MO.  The ladder the employees were using was not extended three feet beyond the roofs edge; it was extended approximately seven inches beyond the edge of the roof.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On 2/22/2017 employees from Angel Martinez-Cabrian were replacing roofing materials on the roof of a residence located at 1349 Cave Springs Estates Drive, St. Peters, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The owner and four employees were observed on the roof of the building approximately nine feet above the ground without fall protection, exposing them to a fall to the ground below.  Angel Martinez-Cabrian was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 760722, citation number 1, item number 2, issued on 4/25/2013, and was affirmed as a final order on 5/17/2013, with respect to a workplace located at 848 Cunningham Ave., St. Charles, MO.
