29 CFR 1910.136(a): The employer did not ensure that each affected employee used protective footwear when working in areas where there was a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet were exposed to electrical hazards:      Shop: Employer is exposing employees to hazardous conditions.  The employees are working without protective footwear   where their feet are exposed to falling objects.   On and about following dates 02/23/2017, 03/21/2017, and 03/30/2017 employees lifted sheets of plywood 8 feet by 4 feet onto a table 38 inches above workplace floor and also lifted 5 gallons containers of liquid materials onto a table 28 inches above the workplace floor.

29 CFR 1910.176(a): Permanent aisles or passageways were not appropriately marked:     Shop: Employer is exposing employees to struck-by hazards.  The employer is using a forklift for material handling purposes in a plant where permanent aisles and passageways were not marked.       On and about the following dates 02/23/2017, 03/21/2017, and 03/31/2017, employees worked where a     Nissan 50 forklift was used for handling materials such as, but not limited to, sheets of plywood where permanent aisles and passageways were not marked.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a power industrial truck:     Establishment: The employer is exposing employees to hazardous conditions.  Employees who are not trained and certified are permitted to operate a forklift in the establishment.    On and about the following dates 02/23/2017 and 03/21/2017 employees operated a Nissan 50 forklift without having received the required training.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     Establishment: Employer is exposing employees to hazardous conditions where employees who use hazardous chemicals. The employees are not provided a written hazard communication program which describes what the labeling system is, what the safety data sheets are, and what training is required.     On and about 02/23/2017 employees worked with hazardous chemicals such as, but not limited to, Acetone without the required written hazard communication program which spelled out how the label and training requirements would be met.
