Lower level employees were not utilizing head protection while working beneath the upper level employees (siding subcontractor) who is handling portable hand tools and installing hardi-plank siding, above the lower level employees, on or about 02/23/17.

Employees were in the process of installing hardi-plank siding to the exterior side of the newly constructed home while working at the edge off of an aluminum pump jack scaffold approximately 20 feet above ground level, without utilizing a means of fall protection, on or about 02/23/17.
