29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): On or about 2/23/2017, during commercial waterline installation, located at the corner of Keynote Circle and Lancaster in Brooklyn Heights, OH, the employer failed to ensure that the employees working in a trench greater than 5 feet in depth were protected from cave-ins by utilizing benching, sloping, or a shielding system in accordance with 29 CFR 1926.652(c).
