On or about February 23, 2017, employees were not provided hard hats while working in a trench where overhead hazards existed, exposing employees to struck-by hazards. On or about February 23, 2017, employees were not provided with a safe means of egress while working in a 7.6 foot deep trench, exposing employees to the hazards associated with not having a safe means of egress in a trench excavation.
