29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) shall be restrained from tipping by guying, tying, bracing, or equivalent means, as follows:    Location:     292 6th Street, Brooklyn NY. Front of Building.  a)     The  4 section high tubular welded frame scaffold was not restrained from tipping by guying, tying or equivalent means. Employees used the tubular welded frame scaffolds to scrape the front exterior façade of the building and it was not restrained by any means. On or about 02/23/2017.

Front of Building.  a)    Employees climbed the cross braces and frames to access the work areas when working from the tubular welded frame scaffold to scrape the front exterior façade of the building.
