29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about February 23, 2017 during the structural framing and installation of roofing material at the job site located at 415 Market St, East Liverpool, Ohio 43920; the employer did not ensure employees working on the 3rd floor of a new construction commercial/residential facility were protected from possible danger of eye injury from impact, or from flying objects from the employees installing structural framing material.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about February 23, 2017, during the installation of framing material at a job site located at 415 Market St, East Liverpool, Ohio 43920; the employer did not ensure employees working installing framing material on the 3rd floor of a new construction commercial/residential facility were protected from a fall of approximately 26.3 feet by utilizing a fall protection system.
