29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about February 23, 2017, employees were exposed to fall hazards greater than 6 feet to the paved ground below, while installing shingles on the roof of a residential building without being trained to recognize fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Artessa Apartment Roof: On or about February 23, 2017, employees without fall protection were exposed to fall hazards greater than 6 feet to the paved ground below while replacing shingles on the apartment roof. Fabian Cadena-Morales was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 917635, citation number 01, item number 001 and was affirmed as a final order on 10/01/2013, with respect to a workplace located at 8401 NE 140th St. Edmond, OK 73025.
