On or about February 23, 2017 and at times prior there to, at the worksite located at 9517 Cliffside Drive, Sherwood, Arkansas, employees were using pneumatic nail guns while preforming framing construction on a new single family home without eye protection. This condition exposed employees to struck by hazards.

This condition exposed employees to fall hazards.
