29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Employees were exposed to amputation hazards on the Duct Line.  Guarding was not provided for the Chicago Dreis & Krump Cross Brake.  There was no point of operation guarding for the Chicago Dreis & Krump Cross Brake.    b) Employees were exposed to amputation hazards on the Duct Line. Guarding was not provided for both Lock Formers. There was no point of operation guarding for both Lock Formers.     (c)  Employees were exposed to amputation hazards when operating the Bridgeport Milling Machines.  There was no point of operation guarding for the rotating chuck and the unused portion of the drill bit. The Bridgeport Milling Machines are located in the Shear Shop.    (d) Employees were exposed to amputation hazards from the 500 Machine manufactured by Vicon Machinery.  The shear part of the 500 Machine was not guarded.  The 500 Machine is located in B Side Fittings.    (e) Employees were exposed to amputation hazards from the Old Collar Line Machine manufactured by Vicon Machinery.  The shear part of the Old Collar Line was not guarded.  The Old Collar Line Machine is located in B Side Fittings.

29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s):    Employees were exposed to amputation hazards from the Diamond Part Revolution Mechanical Power Press in B Side Fittings.  The guard did not prevent entry of hands or fingers in to the point of operation.  The guard extended over the control panel where employees had to reach behind the guard to operate the machine.  The guard also had openings in the top, and bottom front of the guard that an employee could reach into the point of operation.

29 CFR 1910.217(e)(1)(i): The employer did not maintain a certification record of inspections which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the power press that was inspected.    B Fittings Area, employees were exposed to amputation hazards as a certification record of inspections for mechanical power presses was not maintained.  There were no inspection records for the Diamond mechanical power press, the Niargara mechanical power press, and the Alva Allen BT 12 mechanical power press.  There was no documentation that regular inspections of the mechanical power presses were being conducted.

The flywheel for the Alva Allen Mechanical Model BT12 part revolution power press in the Crank Shaft Area was not adequately guarded.  The flywheel was not fully guarded within seven feet of the floor.    (b)  Employees were exposed to amputation hazards.  The vertical band saws in the Shear Shop both had a portion of the flywheel which was unguarded.  The flywheel was not fully guarded within seven feet of the floor.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:    (a) Employees were exposed to amputation hazards from the 500 Line Vicon Machinery in B Side Fittings.  The rotating shaft near the end of the machine where the belt and pulleys are connected was not adequately guarded.  On or about February 3, 2017, an employee received an amputation when his hand got caught in the rotating shaft, and belt and pulley at the end of the machine.    (b) Employees were exposed to amputation hazards from the 500 Line Vicon Machinery in B Side Fittings.  The rotating shaft at the very end of the machine that forms the collars was not adequately guarded.  The outgoing end is unguarded.    (c) Employees were exposed to amputation hazards from rotating shafts on the Old Collar Line Roller Feed Assembly in B side Fittings.  The rotating shafts had a guard which was inadequate.  There was an opening in the side exposing employees to the rotating shafts.    (d) Employees were exposed to amputation hazards from rotating shafts on the Shear to Length Line.  There were ingoing nippoints from the rotating shafts at the beginning of the machine after the decoiler, and at the end of the machine.  There were no guards to protect employees from the ingoing nippoints.

Employees were exposed to ingoing nippoints from the belts and pulleys which were not adequately guarded within seven feet of the floor.  Conveyor sections of the 500 machine did not have the pulleys adequately guarded. On or about February 3, 2017 an employee received an amputation injury of their finger when their hand got caught in the rotating shaft, and horizontal belts and pulleys while operating the 500 machine.    (b) Employees were exposed to amputation hazards from the Old Collar Line manufactured by Vicon Machinery.  Employees were exposed to ingoing nippoints from the belts and pulleys which were not adequately guarded within seven feet or less from the floor.  Conveyor sections of the Old Collar Line did not have the pulleys adequately guarded.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    (a) Employees were exposed to electrical shock in the B Fittings Area.  The Diamond part revolution mechanical power press did not have adequate strain relief.  The electrical flexible conduit did not have adequate strain relief as the fitting had come apart exposing electrical wiring.    (b) Employees were exposed to electrical shock in the Pipe Line Area.  The Milwaukee Drill did not have adequate strain relief.  The electrical cord next to the body of the drill had a broken strain relief exposing interior wiring.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.     Hammond Sheet Metal dba Barrington Manufacturing Company of El Dorado Springs, Missouri did not report an amputation as a result of a work-related incident to OSHA within twenty-four hours.  An incident occurred on or about February 3, 2017 which involved a partial amputation of a finger. This incident was not reported to OSHA.
