29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Breaking Room - On or about 2/22/2017, the employer did not ensure that a pump was properly isolated from its energy source. Employees were exposed to the unexpected start up of the pump when completing daily clean out procedures on the pump without using lockout and/or tagout devices.
