29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about 2/22/17, at the theater area of the site, Niagara Falls, NY. The plug of a ventilation fan lacked the ground prong. Employees were exposed to shock when operating the fan. NO ABATEMENT CERTIFICATION REQUIRED
