29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:  On or about 2/23/2017 , on the back side roof of a two stories house plus basement;  Three employees engaged in roof renovation activities were exposed to fall hazards through two unprotected skylight holes, up to 14 ft height from the ground level.
