Jobsite: Basement, south section - Employees installing fire sprinkler pipes were exposed to fall hazards while working in close proximity to floor holes with unsecured covers. An employee removed an unsecured floor hole cover and fell through the floor hole to the sub-basement 10 feet below. Noted on or about 2/21/17.
