29 CFR 1910.119(g)(1): The employer did not provide initial training on operating procedures for employees required to work on or near a covered process. Maintenance employees, working on the ammonia refrigeration system, were not adequately trained on safe work and operating procedures such as emergency evacuation procedures and management of change procedures to be followed, exposing them to hazards of ammonia inhalation and chemical burns.

29 CFR 1910.119(n): The employer did not establish and implement an emergency plan for the entire plant in accordance with the provisions of 29 CFR 1910.38: The emergency response plan for releases of ammonia did not instruct employees on how to distinguish between small (level 1) and large (level 2) releases of ammonia and adequate training on assembly areas was not provided, thereby exposing employees to hazards of ammonia inhalation and chemical burns.
