29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a) Boiler room: On or about October 7, 2016 the employer did not report the accident of an employee resulting in an in-patient hospitalization, and a broken left wrist, and left shoulder due to a fall onto concrete ground.  Note* abatement certification IS required for this item.

29 CFR 1910.1200(h)(3)(iv): Employee training did not include the details of the hazard communication program developed by the employer, including an explanation of the labeling system and the material safety data sheet, and how employees can obtain and use the appropriate hazard information:  The Riverside Hotel: On or about February 24, 2017 and at times prior thereto; employer did not provide sufficient GHS and hazard communication training to staff and employees.  Material Safety Data Sheets (MSDSs) have not been replaced by Safety data Sheets (SDSs). Riverside is a multi-lingual workplace and GHS requirements have not been adequately conveyed or implemented.      Note* Abatement certification IS required for this item.
