Fabrication Area:  On, or about, February 24, 2017, safeguarding was not provided on the Chicago, Dreis and Krump mechanical press brake.    D.  Fabrication Area:  On, or about, February 24, 2017, the barrier guard provided on the Niagara ten foot shear did not protect the hold-downs and allowed access to the shear point under and through the guard.
