29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Jobsite, 520 Oakridge Drive, Johnstown, PA 15904: On or about February 24, 2017, during box manufacturing operations on the left hand box cutting machine and the gluer machine, the employer did not ensure that the machines were turned off and the energy source isolated, therefore exposing employees to amputation type injuries while employees were resetting these machines due to a temporary loss of power.
