29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:  On or about February 23, 2017, and times prior thereto, throughout the newly built commercial building where employees were exposed to impalement hazards when performing steel erection without all protruding reinforcing steel being guarded.

29 CFR 1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section.  On or about February 23, 2017, in the middle area of a commercial building being erected where employees were exposed to a fall hazard exceeding 15 feet when performing connecting work without wearing fall protection equipment.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about February 23, 2017, on the west side of the construction project where employees were exposed to a fall hazard when using Werner extension ladders to access the upper steel beams without the ladders extending at least three feet above the working surface.
