29 CFR 1926.34(c): "Maintenance and workmanship." Means of egress shall be continually maintained free of all obstructions or impediments to full instant use in the case of fire or other emergency. 1) Jobsite: Employees did not have a clear unobstructed means of egress from the top of a scaffold. Employees installing roof shingles were not provided a safe means of egress in that debris and materials were obstructing the path exposing employees to injuries in the event of an emergency. Condition was observed on or about 1 FEB 2017

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. 1) Jobsite: Employees were exposed to holes on the top level of a scaffold. Employees were not provided with a safe walking/working surface in that floor holes, ranging from 5 1/2 to 11 inches were not covered; thus exposing employees to trip and fall hazards. Condition was observed on or about 1 FEB 2017.
