29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. (a) Location: 77 Court Street, West Newton, MA On or about 02/23/3017 employees were working on an Alum-A-Pole pump jack scaffolding with only one ridged triangular brace per pole installed at the roof edge which was greater than 30 feet in height.
