29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Naval Weapons Station Yorktown: On or about 02/23/2017, an employee walked on the ridge of a 5 in 12 sloped roof, with unprotected sides and edges, approximately 36 ft. above the ground, and was not protected from a possible fall. No abatement certification or documentation required.

29 CFR 1926.502(d)(6)(i): A snaphook not of the design type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other; (iii) to a Dee-ring to which another snaphook or other connector is attached; (iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself: (a) Naval Weapons Station Yorktown, BEQ: On or about 02/23/2017, an employee used a rope lanyard with a snap hook that was not designed to be connected directly to the horizontal lifeline as a result employee was exposed to a 36 ft. fall hazard. No abatement certification or documentation required.

29 CFR 1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training. a) Naval Weapons Station Yorktown: On or about 02/23/2017, the employer did not verify compliance with paragraph (a) of this section by preparing a written certification record as required. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
