29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) On or about 02/15/2017 - 499 Boeing Boulevard, Huntsville, AL, an employee entered a collapsed trench measuring approximately 27 feet long x 15 feet, 3 inches wide and approximately 4 feet, 6 inches deep with vertical walls and on slightly sloping ground to assist in rescuing a trapped employee.
