29 CFR 1910.305(j)(1)(iii): Lampholders installed in wet or damp locations were not the weatherproof type: (a) Stone Valley Pet Lodge - Employees were exposed to electric shock from the grooming station which was equipped with two screw-shell lampholders that were not grounded or of the waterproof type, on or about February 28, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (a) Stone Valley Pet Lodge - The employer did not develop or implement a written hazard communication program for the employees using acetic acid and ethyl alcohol (eye, skin, and nose irritants) to clean the facility, on or about February 28, 2012. Abatement certification required with 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
