29 CFR 1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard: On or about February 25, 2017, and times prior to, in the Wet Section Pre-Treat area, standard railing was not provided on the platform that employees accessed to clean and change out rollers on the Chem Coater machine. Employees were exposed to falls.

29 CFR 1910.147(a)(2)(ii): Alternative measures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by the exception of 1910.147(a)(2)(ii) for routine, repetitive, and integral processes such as tool change, change over, and cleaning: On or about February 25, 2017, in the Wet Section Pre-Treatment area, adequate training and procedures had not been established to control all hazardous energies to the Chem Coater machine when employees performed roller cleaning tasks. Failure to do so exposed employees to the hazard of contact with nip points and amputations.
