29 CFR 1910.1200(f)(6)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals. a) Chemical Storage Area - On or about 08/22/2017, employees were exposed to severe skin burns and eye damages, and respiratory tract irritations when employer did not ensure that a container of hazardous chemical, Polycat 37 Catalyst, was labeled and marked with the chemical name and hazard pictograms.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. a) Throughout Production department - On or about 2/28/2017, employees were exposed to fire hazards, in that portable fire extinguishers were not mounted and identified for employees working with flammable materials such as, but not limited to DABCO B-16 Catalyst (class 2 flammable) and propane gas (highly flammable).
