Avon, OH: The employer did not have a plan or program ensuring frequent and regular inspections of the job site are performed by a competent person. Such inspections would provide identification and correction of common job site hazards such as falls and exposures during job site operations, decreasing the chance of employee injury and/or illness.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Tall Order LLC on or about February 27, 2017, at the address near the 3600 block of Nagle Rd. Avon, OH: The employer did not ensure employees were protected from falls while installing new roofing material atop a residential type structure. Employees working in this condition were exposed falls of 11 ft. fall to the ground below. Bob Rising of Tall Order LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1176925, citation number 1, item number 1b and was affirmed as a final order on October 27, 2016, with respect to a workplace located at the northeast corner of Greystone Rd. and Middle Ridge Rd. Amherst, OH.

Bob Rising of Tall Order LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1176925, citation number 1, item number 1c and was affirmed as a final order on October 27, 2016, with respect to a workplace located at the northeast corner of Greystone Rd. and Middle Ridge Rd.
