29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.  WORKSHOP:  The rotating pulleys of the following sewing machines were unguarded:    1.    Singer sewing machine & Jado Sewing Machine; Plate Number AG412604 & G6083836.      2.    Singer sewing machine; Plate Number AG416272,  3.    Singer sewing machine; Plate Number; AG427212.  4.    Singer sewing machine; Plate Number G6083813. ABATED DURING THE INSPECTION.    5.    Singer Sewing machine; Plate Number AG427239. ABATED DURING THE INSPECTION.    4.      Consew sewing machine, plate number SL0910006. ABATED DURING THE INSPECTION.   5.      Singer sewing machine, Cam-matic 191, plate Number AD892249, ABATED DURING THE INSPECTION.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section.  WORKSHOP: The inclined horizontal belts of the following sewing machines were partially unguarded:    1.      Singer sewing machine; Plate Number G6083813     2. Vertical belts running over a lower pulley more than seven (7) feet above floor or platform shall be guarded at the bottom in the same manner as horizontal overhead belts, if conditions are as stated in paragraphs (e)(2)(ii) (a) and (c) of this section.  WORKSHOP:  The vertical belts of the following sewing machines were unguarded:  1.    Singer sewing machine, Jado Sewing Machine; Plate Number AG412604  2. 29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  WORKSHOP:  The employer had not verified through a written certification that the workplace hazard assessment for personal protective equipment (PPE) had been performed as complete.

29 CFR 1910.133(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.  WORKSHOP: The employer had not ensured that all affected employees, who were wearing prescription glasses were wearing appropriate types of eye protection safety glasses with side shields.
