29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:    (a) On or about 3/1/2017, employees of Jetro Holdings, LLC. working in the warehouse and cashier areas at 101-10 Foster Ave were exposed to fire hazards because the employer had not ensured that employees were able to open an exit route door from the inside at all times without keys, tools, or special knowledge. The following are instances.     (1) on the southeast side of the building, for an exit door where two locks and a manager's key were necessary to open the door    (2) on the northwest side of the building, for an exit door where a slide bolt was installed approximately 2' above the floor and required for bypass    (b) On or about 3/28/2017, employees of Jetro Holdings, LLC. working in the warehouse and cashier areas at 101-10 Foster Ave were exposed to fire hazards because the employer had not ensured that employees were able to open an exit route door from the inside at all times without keys, tools, or special knowledge. The following are instances.     (1) on the southwest side of the building, for an exit door where a slide bolt was installed approximately 2' above the floor and required for bypass    (2) on the northwest side of the building,  for an exit door where the door had been padlocked with an unknown key, attached to a slide bolt    (3) on the northwest side of the building,  for an exit door where the door had been physically cemented shut    (4) on the southeast side of the building,  for an exit door where a slide bolt was installed approximately 4' from the floor and required for bypass

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    (a) On or about 3/1/2017, employees of Jetro Holdings, LLC. working in the warehouse and cashier areas at 101-10 Foster Ave were exposed to fire hazards because the employer did not ensure that exit routes were kept free and unobstructed. The following are instances.    (1) on the southeast side of the building, where the exit route leading to an exit door was obstructed by pallets    (2) on the southeast side of the building, where the exit access in front of an exit door was partially obstructed by equipment and shelving    (3) on the northwest side of the building, where the exit discharge for an exit door was blocked by heavy meal equipment    (b) On or about 3/28/2017, employees of Jetro Holdings, LLC. working in the warehouse and cashier areas at 101-10 Foster Ave were exposed to fire hazards because the employer did not ensure that exit routes were kept free and unobstructed. The following are instances.     (1) on the northwest side of the building, where the exit route leading to an exit door was obstructed by a locked, chain-link fence and door    (2) on the northwest side of the building, where the exit access to a exit door was obstructed by a bicycle that was chained and locked to a bolt on the door

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":    (a) On or about 3/1/2017, employees of Jetro Holdings, LLC. working in the warehouse and cashier areas at 101-10 Foster Ave were exposed to fire hazards because the employer failed to ensure that emergency exits were signed with a red sign marked "Exit". The following are instances.    (1) for an exit door on the southeast side of the building  (2) for an exit door on the southeast side of the building    (b) On or about 3/28/2017, employees of Jetro Holdings, LLC. working in the warehouse and cashier areas at 101-10 Foster Ave were exposed to fire hazards because the employer failed to ensure that emergency exits were signed with a red sign marked "Exit". 29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:    (a) On or about 3/1/2017, employees of Jetro Holdings, LLC. working in the warehouse and cashier areas at 101-10 Foster Ave were exposed to fire hazards because the employer did not ensure that signs were posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent. The following are instances.    (1) at the intersection of the aisleway parallel with the southeast wall near an exit door and Aisle 3    (2) along the southeast wall, near pallet storage    (3) in the passageway between two exit doors, an interior door and one opening to the building's exterior  (4) at the intersection of the aisleway parallel with the southeast wall near and exit door and Aisle 4    (b) On or about 3/28/2017, employees of Jetro Holdings, LLC. working in the warehouse and cashier areas at 101-10 Foster Ave were exposed to fire hazards because the employer did not ensure that signs were posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent.
