29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail Systems, Safety Net Systems, or Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. a. On or about February 17, 2017, on the lavatory service truck, while servicing the 2nd aircraft lavatory drain port, an employee standing on the aft platform was exposed to a 5 feet high fall hazard from the unprotected edges of the walking-working surface of the back of the truck.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a. On or about February 28, 2017, in and around Building 968 and the flight line on Eglin AF, the employer did not perform a hazard assessment regarding potential exposure to struck-by or crushing injuries to employees' feet and the need for protective footwear when servicing aircraft, operating forklifts, and working near Utility Task Vehicles and ground support vehicles.
