Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:     a) Shop Area - On or about December 28, 2016, an employee was exposed to a fall of approximately 10 feet when the employee was raised by a Clark ECG-25 forklift while standing on a pallet without any means of fall protection.    Among other methods, one feasible and acceptable means of abatement to correct this hazard is to provide and require the use of a work platform designed to lift personnel in accordance with the American Society of National Standard (ANSI), B56.1-2012 Safety Standard for Low Lift and High Lift Trucks, Section 4.17.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system:    a) Shop Area - On or about December 28, 2016, an employee was exposed to a fall of approximately 14 feet when the employee unhooked chains for the crane from the top of a boiler without any means of fall protection.    b) Shop Area - On or about December 28, 2016, an employee was exposed to a fall of approximately seven to eight feet high when removing a flange on the boiler pipes.
