29 CFR 1910.1048(k): When there was the possibility of an emergency involving formaldehyde, the employer did not ensure that appropriate procedures were adopted to minimize injury and loss of life and implemented in the event of an emergency: Surgical Services - Effective emergency procedures regarding formalin, a formaldehyde 10% solution, were not established for employees to use to minimize injury in case of an emergency spill of the formalin when dispensing this chemical to cover and fix specimens or changing the five gallon carboy of formalin in the fume hood.
