29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):  The employer failed to protect employees from in-running nip points.  The most recent occurrence was observed at 1838 N. Broadway, St. Louis, MO 63102 as the belt and pulley drives of the vehicle pressure wash pumps were not guarded in the six mobile wash units.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  Location:  1838 North Broadway, St. Louis, MO  The employer failed to ensure that all containers of hazardous chemicals were labeled in the workplace.  An approximate 350 gallon dispensing drum and the approximate 5 gallon containers of corrosive degreasing Scrubby "B-Z" were missing product identifier labels.

29 CFR 1910.1200(h)(3)(iv): Employees had not been provided with GHS 2012 training including an explanation of the labels received on shipped containers and the workplace labeling system to be used by their employer; the safety data sheet, including the order of information, and how employees can obtain and use the appropriate hazard information:  The employer did not provide adequate hazard communication training to include new labeling elements and the new safety data sheet format.  Employees utilize a variety of vehicle cleaning products to include, but not limited to:   a)  738 Citrol X b) Scrubby B-Z c) Z-05-723 Aluminum Brightener d) Scrubby Hi-Pressure Washing Compound
