29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization of one or more employees to OSHA within twenty-four (24) hours. Facility - On or about November 21, 2016, an employee fell from a ladder breaking his arm and was hospitalized for surgery to repair it. The employer did not report this incident to OSHA until February 22, 2017.
