29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)  North side, entrance overhang: Three employees exposed to head injury while working on the entrance overhang without any head protection; using hammers and nail guns to attach framing lumber and standing under other employees working above them; on or about 02/24/2017;  b)  North side, entrance overhang: Employee without head protection was holding the 6' step ladder while another employee climbed the ladder with an approximate 29 foot long 2x10 inch plank on his shoulder, employee climbing ladder not using any head protection, on or about 02/24/2017;   c)  North side, west end of building; exterior wall of the Pro-Shop:  Employee without head protection and working from an improperly set up extension ladder was using a pneumatic nail gun while attaching sheeting above the windows, on or about 02/24/2017. 29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it:   a)  North side of entrance overhang: Employees without any fall protection worked on the underside of the entrance overhang from the makeshift scaffold fabricated from left over lumber at heights between approximately 9 feet 10 inches and 11 feet, on or about 02/24/2017;   b)  North side of entrance overhang: Employee without any fall protection approximately 18-20 feet above ground level, without any type of fall protection, was standing on top of an unsecured sheet of OSB resting on the forks of the Model B-66 Pettibone Telehandler while removing construction debris from the roof of the overhang, on or about 02/254/2017.
