29 CFR 1910.179(j)(3): A complete periodic inspection of crane(s) had not been conducted in the past 12 months: Hoist Room: On or about February 14, 2017, the employer did not ensure that a yearly inspection was done on the 1.5 ton Omni overhead crane, serial #12883. The previous inspection expired in August 2016.

29 CFR 1910.179(l)(3)(iii)(a): Crane hooks showing defects described in paragraph (j)(2)(iii) of this section shall be discarded. Repairs by welding or reshaping are not generally recommended. If such repairs are attempted they shall only be done under competent supervision and the hook shall be tested to the load requirements of paragraph (k)(2) of this section before further use. Hoist Room: On or about February 14, 2017, employees were allowed to operate an Omni 1.5 ton overhead crane, serial #12883, that had a defective crane hook. The crane hook was missing the throat latch.
