OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards while using automotive lifts:    (a)  On or about March 2, 2017, and at times prior, the employer did not ensure that employees were safe from crushing and struck-by hazards while using wood blocks on the following auto lifts:    (1)  Hunter Alignment Machine, in Bay 4 of the Service Department.  The wood blocks are used when the lifting arms do not lift high enough for the work that needs to be performed.    (2)  The Tire Chief Lift in Speed Lane Bay 1. The wood block is used to help balance long vehicles.    (b)  On or about March 2, 2017, and at times prior, the employer did not ensure employees were safe from crushing and struck-by hazards when riding in automobiles as they were lifted by the two-post Challenger Lift in the Speed Lane Bay 2, and then exiting the raised vehicle.        (c)  On or about March 2, 2017, and at times prior, the employer did not ensure employees were safe while using the Hunter Alignment Machine, in Bay 4 of the Service Department in that it had not been inspected for 18 months. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a) On or about March 2, 2017, and at times prior, employees working in Bay 1 of the Service Department were exposed to electrical hazards from flexible cord terminal box that was designed for permanent mounting   and was being used to power the tire machine. 29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    (a)  On or about March 2, 2017, and at times prior, employees were exposed to shock, burns, and fire hazards in Bay 1 of the Service Department in that the insulation of the power cord to the tire machine was separated from the plug, causing strain to be placed on the live conductors. 29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:      (a) On or about March 2, 2017, and at times prior, employees who handle used car batteries were exposed to eye injuries at the car battery storage area in the event of splash or spill of battery acid.
