29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:      On or about March 16, 2017, in the plant production area, where employees were exposed to noise hazard without a continuing, effective conservation program in place;    The Inline Thermoform Press machine operator was exposed to a noise level of 88.98 dBA during a 381 minutes sampling period on March 16, 2017.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      At this establishment, in the plant production area,  where employees were exposed to struck-by, caught-in, and electrical hazards when performing service and/or maintenance, energy control program with all required elements was not established by the employer for the following types of machines:    a. Sencorp Inline Thermoform Press Machine Automatic System 1;  b. Sencorp Inline Thermoform Press Machine Automatic System 2;  c. Router Dual Table Machine System;  d. Router Single Table Machine System;  e. Tronomatic Press Machine Automatic System;  f. Maac Single Station;  g. DT Industries Plastics & Packaging Machine Automatic System; and,  h. Cam Single Station.
