Stark Avenue in Kansas City, MO: Employees engaged in residential roofing activities from a 6:12 pitch roof were exposed to fall hazards of approximately 12 feet from the eave of the residence to the ground below in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about March 3, 2017 at a jobsite located at 10058 N. Stark Avenue in Kansas City, MO: Employees were exposed to injury from an 12 foot fall hazard while accessing and exiting the roof. The ladder that provided access to the roof on the front of the house did not extend 3 feet above the eave.
