29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) Storage Products Group, LLC located at 14002 E. 33rd Place Aurora, CO 80011: On and before February 7, 2017, the employer did not ensure procedures were developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in die setting operations on six different press brakes.  Operators placed their hands in the point of operation while performing die setting without utilizing the hazardous energy control procedures developed for each machine. The employees used press brakes to bend metal hardware products.  This condition exposed employees to an amputation hazard.    Abatement Note:  Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:      1)     A specific statement of the intended use of the procedures;      2)     Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;      3)     Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and      4)     Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

Abatement Note:  The training shall include the following:    1)     Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.    2)     Each affected employee shall be instructed in the purpose and use of the energy control procedure.    3)     All other employees whose work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or re-energize machines or equipment which are locked out or tagged out.

29 CFR 1910.212(a)(3)(ii): Point of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:    (a) Storage Products Group, LLC located at 14002 E. 33rd Place Aurora, CO 80011: On and before February 7, 2017, the employer did not ensure point of operation on five hydraulic press brakes were guarded to prevent employees from having any part of their body in the danger zones during operating cycles.  Operators placed their hands within two inches of the point of operation while holding material during press brake operation. The employees used press brakes to bend metal hardware products. This condition exposed employees to an amputation hazard.    (b) Storage Products Group, LLC located at 14002E. 33rd Place Aurora, CO 80011: On and before February 7, 2017, the employer did not ensure point of operation on a Cincinnati Shear model 1812 was properly guarded in accordance with Subpart O, Table O-10.  The measured distance of 3 inches from the bottom edge of the guard that was installed to the point of operation required an opening of no greater than 1/2 inch.  The opening was approximately 11/16 inches.  This condition exposed employees to amputation hazards.
