29 CFR 1910.217(c)(1)(i): The employer did not provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. a) Fabrication Area / Southeast: The point of operation of the 35 Ton Axle Press was not guarded when employees cut metal axles 7 1/8" - 9" in length, on or about 2/21/17. An employee's finger was partially amputated when it was crushed between the ram and the die.

29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment: a) Facility: The employer did not maintain certification records of inspections for the axle press, on or about 2/28/17. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
