29 CFR 1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. a) Production and Warehouse Areas: On or about February 14, 2017, and at times prior, the employer failed to certify that operators of Crown and Raymond forklifts had been trained and evaluated in accordance with the OSHA standard. The employer exposed employees to struck-by injuries if the vehicles were operated unsafely while performing work at the industrial site.
