29 CFR 1910.145(c)(2)(i): Caution signs were not used to warn against potential hazards or to caution against unsafe practices: a) Fast Food Line 3; On or about April 2, 2017 and at times prior thereto, the employer exposed employees to amputation hazards in that the employer failed to use a caution sign or equivalent to warn employees of potential amputation hazards from the tail cutter on including but not limited to Fast Food Line 3.
