29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)  Medford Village Country Club, roof and adjacent areas below eaves:  Employees working at different levels, without any head protection, on the 12/12 and 4/12 pitch roof installing roofing felt with staple slappers and shingles with pneumatic nail guns, on or about 02/24/2017.   NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.102(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable:  a)  Medford Village Country Club, on the roof and on the ground below the eaves: Employees without any fall protection installing roofing felt and shingles, exposed to a fall height of approximately 11-16 feet from the roof eaves, were using hammers, staple slappers, and pneumatic nail guns without any eye protection, on or about 02/24/2017.   NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a)  North side of entrance overhang on roof: Employees approximately 16 feet or greater above the ground and without any type of fall protection were applying roofing felt and shingles to the 12/12 pitched roof, on or about 02/24/2017;  b)  North side roof for Pro Shop: Employees approximately 16 feet or greater above the ground and without any type of fall protection were applying shingles at the eave to the 12/12 pitched roof, on or about 02/24/2017.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  North side of the Medford Village Country Club, north side of entrance overhang:  Employees accessed the 12/12 pitched roof by using an improperly setup aluminum extension ladder; ladder extended approximately 1 foot above the eave of the roof, on or about 02/24/2014.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
