29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or likely to be present, which necessitate the use of personal protective equipment (PPE):      a) Chemical Room(Ink Room) and Pre-Press Room: On or about July 11, 2017, and at times prior, the employer exposed employees to chemical hazards in that the  employer failed to require the proper personal protective equipment (PPE) for employees working with hazardous products such as Film Kleen 7065 (Antistatic Film Cleaner),  DT-2R Plate Developer Replenisher, DT-2W Plate Developer , MRC-F, and Fujifilm Ink Anti-Skin LB.      b) Press #1 Paper Feeder and Press #2 Paper Feeder: On or about July 11, 2017, and at times prior, the employer exposed employees to struck- by hazards in that the employer failed to require the proper personal protective equipment (PPE) while working within the moving range of the paper feeder using the power pallet lift and loading pallets weighing approximately 2000 lbs.

29 CFR 1910.151(c): Where the eyes of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes were not available for immediate emergency use.    a) Pre-Press Room - Kodak Trendsetter: On or about July 11, 2017, and at times prior, the employer exposed employees to burns to eye and skin hazards causing serious injury and impairment in that there are not appropriate facilities to quickly flush the eyes when using caustic chemicals with a pH greater than 10 such as  DT-2R Plate Developer Replenisher and DT-2W Plate Developer .
